The subject of this thesis deals with the analysis of two rail accidents in the UK using the STAMP and ACCIMAP methodologies. The first accident is the Selby Rail Disaster and occurred at Great Heck on 28 February 2001, which involves the derailment and collision of two trains caused by a vehicle entering the rail yard and the first train colliding with it. The second accident concerns the Grayrigg accident which occurred on 23 February 2007, the derailment of which was caused by a fault in a moving part of the railway line. The accidents were caused by human error, which is the result of using the two methods for the systemic analysis and mapping of incidents. The purpose of the thesis is to examine the problems that exist in the application of these investigation techniques and to propose a way to integrate these two techniques in the same analysis framework.
