Summary

The objective of this thesis is to highlight the latent failures of an Airline Organization performing operational flights, which are nested within its existing structures and processes and can create large-scale accidents despite sophisticated safety management systems. The methodology that will be used is the Accimap accident analysis method which identifies the causal chains of accidents of complex socio-technical systems related to the Organization's Management and its hierarchical structure.At the beginning, a theoretical approach will be made to the concepts of the State as it is the main provider of the resources of the State structures to operate the Public activities which are necessary for its public policy. For the orderly operation of these, appropriate management strategies are required which derive from the State Constitution. The Rules and procedures of Public Organizations are included and are inextricably intertwined with the Laws deriving from the Constitution and State Institutions, which will be analyzed below. Finally, an analysis will be made of the structure and responsibilities of the Governmental Institution as it is the basis for the implementation and control of the enacted Laws.Subsequently, the theory of the structure of Public Administration will be analyzed as this is of global importance in every process of a Public Organization. An analysis will be made in the behavioral form of direction and utilization of the human resources of the multi-level purpose of the Administration as this is the governmental mechanism of the State where it applies the interwoven Rules and Laws arising from the Constitution and its Institutions.Continuing, there will be a segmentation of the structures of a Public Organization and how they are affected by its external forces, resulting in the effects of incorrect Administrative decisions, the responsibilities and powers of the Administrative officers by level of Administration and how these are assigned to lower officers without renouncing the corresponding responsibilities. Immediately afterwards, the categories of decision-making are presented in detail and emphasis is placed on the degree of knowledge of the information that should exist in order to know the result of the Administrative decisions and how they affect flight safety in an airline organization when the information is not transmitted according to the loops security check.Finally, there will be an analytical presentation of a typical Safety Management System in an airline organization, how to control it to avoid unwanted events, an analysis of error factors and separation of human from organizational errors and emphasis will be placed on how within an organizational system with strict safety standards, the wrong Administrative decisions affect the entire Safety Management System and this mistake develops into an accidental event.The methodology is applied in three (3) stages. In the first stage, semi-structured interviews will be conducted to determine the qualitative factors-variables that contribute to an air accident. For the analysis of the qualitative data, a Thematic Analysis will be conducted in order to gather a set of categories of factors in order to derive the questions of the quantitative analysis of the research. Atlas.ti.V8 software will be used for qualitative data analysis. In the second stage, a sample survey will be conducted with a closed questionnaire in order to identify the direct and causal-variable factors that are capable of causing the accident. In the third stage, a statistical analysis will be performed where, from the set of variables presented, with an inductive analysis of the data, the variables with the strongest indications will prevail while the rest will be rejected. These variables will then be applied to the Accimap chart in a hierarchical format so that they end up with the accidental event.This methodology can become a guide for prevention and preparation of Safety Rules and procedures by aviation Organizations to include the entire range of operational missions in order to limit all contributors to accidental events and to make safety the Organization's ability to immediately build capacity and effectiveness.Due to Classified information, the survey does not enter into reports of dates, times, names, locations, aircraft types, call characteristics, etc., without this affecting its results.
