Summary

The present thesis investigates the accidents that occurring in quarry sites using the bowtie method. Bowtie diagrams were chosen for the analysis because they show a visualized overview of the hazards, provides all the critical information of risk, such as threats, critical events, consequences, barriers and shows scenarios clearly with a cause-effect logic. They are very understandable by everyone in a company, from senior managers to technical staff.The statistics of recent years in the international, as well as in the Greek quarrying sector, show the dangerous nature of the workplace and the increased probability of an accident, which can lead to serious and fatal injuries, destruction of equipment and materials. Risk identification and risk assessment are essential to reduce accidents and their impact on workers and on organization. Representative accidents occurring at the aggregates quarry, in concrete and asphalt mix production plants, at all stages of operations with use of mobile or fixed equipment, were selected. This analysis has been carried out using the bowtie method and is presented by means of an explanatory diagram. The occurring hazards during the execution of the work in the quarry site, the causes that trigger the hazards and their consequences are analysed. In addition, the barriers that can prevent the threats from occurring and, if a hazard occurs, mitigate the undesirable situations, are presented. The conclusions of the analyses were categorized and presented in summary tables.Results shows that the main causes of accidents are human error, inadequate training of staff and poor maintenance of equipment. The indicated preventive measures are the use of suitable personal protective equipment (P.P.E.) and the adequate training to standard job procedures (e.g. LOTOTO, S.O.P). These measures contributes significantly to prevent accidents. Mitigation of their consequences can be achieved by immediate first aid and isolation of the accident site.
