| Content Summary | Το αντικείμενο της παρούσας μεταπτυχιακής εργασίας είναι να αναλύσει ατυχήματα που συμβαίνουν σε χώρους λατομείου με τη μέθοδο bowtie. Επιλέχθηκε να γίνει ανάλυση με διαγράμματα bowtie επειδή δίνουν μια πολύ κατανοητή απεικόνιση των κινδύνων, παρέχουν όλες τις κρίσιμες πληροφορίες, όπως απειλές, κρίσιμα γεγονότα, συνέπειες, φραγμοί και εμφανίζουν τα σενάρια που εξετάζονται με τη λογική αιτίας-αποτελέσματος. Επιπλέον τα διαγράμματα bowtie είναι πολύ κατανοητά από όλους σε μια επιχείρηση, από τα ανώτερα διευθυντικά στελέχη έως το τεχνικό προσωπικό. Τα στατιστικά στοιχεία των τελευταίων ετών στο διεθνή, αλλά και στον ελληνικό λατομικό κλάδο, δείχνουν την επικινδυνότητα του χώρου και την αυξημένη πιθανότητα να συμβεί κάποιο ατύχημα, το οποίο μπορεί να οδηγήσει σε καταστροφή εξοπλισμού, υλικών και μέσων, παράλληλα με σοβαρούς και θανάσιμους τραυματισμούς. Η αναγνώριση και η αξιολόγηση των κινδύνων και η εκτίμηση της επικινδυνότητας είναι απαραίτητες για τη μείωση των ατυχημάτων και των επιπτώσεών τους στους εργαζόμενους και στις επιχειρήσεις. Επιλέχθηκαν αντιπροσωπευτικά ατυχήματα που συνέβησαν στο χώρο παραγωγής των αδρανών, σε μονάδες παραγωγής σκυροδέματος και ασφαλτομίγματος, σε όλα τα στάδια των εργασιών και με χρήση κινητού ή σταθερού εξοπλισμού. Η ανάλυση με τη μέθοδο bowtie παρουσιάζεται μέσω διαγραμματικής απεικόνισης και εντοπίζονται οι κίνδυνοι που υπάρχουν κατά την εκτέλεση των εργασιών στο λατομικό χώρο, οι αιτίες που ενεργοποιούν τους κινδύνους καθώς και οι συνέπειές τους. Επιπλέον, παρουσιάζονται οι φραγμοί, οι οποίοι μπορούν να εμποδίσουν την εμφάνιση των κινδύνων και να μετριάσουν τις επιπτώσεις. Τα συμπεράσματα των αναλύσεων κατηγοριοποιήθηκαν και παρουσιάζονται σε συγκεντρωτικούς πίνακες. Από την μελέτη των συγκεντρωτικών πινάκων προκύπτει ότι οι κύριες αιτίες των ατυχημάτων είναι το ανθρώπινο λάθος, η ανεπαρκής εκπαίδευση του προσωπικού και η κακή συντήρηση του εξοπλισμού. Τα προληπτικά μέτρα που βοηθούν είναι η χρήση μέτρων ατομικής προστασίας (Μ.Α.Π.) και η εκμάθηση τεχνικών εκτέλεσης συγκεκριμένων εργασιών (LOTOTO, S.O.P). Οι ενέργειες αυτές βοηθούν στην πρόληψη των ατυχημάτων. Στον μετριασμό των επιπτώσεών τους βοηθάει η άμεση παροχή πρώτων βοηθειών και η απομόνωση του χώρου του ατυχήματος. | el |
| Content Summary | The present thesis investigates the accidents that occurring in quarry sites using the bowtie method. Bowtie diagrams were chosen for the analysis because they show a visualized overview of the hazards, provides all the critical information of risk, such as threats, critical events, consequences, barriers and shows scenarios clearly with a cause-effect logic. They are very understandable by everyone in a company, from senior managers to technical staff. The statistics of recent years in the international, as well as in the Greek quarrying sector, show the dangerous nature of the workplace and the increased probability of an accident, which can lead to serious and fatal injuries, destruction of equipment and materials. Risk identification and risk assessment are essential to reduce accidents and their impact on workers and on organization. Representative accidents occurring at the aggregates quarry, in concrete and asphalt mix production plants, at all stages of operations with use of mobile or fixed equipment, were selected. This analysis has been carried out using the bowtie method and is presented by means of an explanatory diagram. The occurring hazards during the execution of the work in the quarry site, the causes that trigger the hazards and their consequences are analysed. In addition, the barriers that can prevent the threats from occurring and, if a hazard occurs, mitigate the undesirable situations, are presented. The conclusions of the analyses were categorized and presented in summary tables. Results shows that the main causes of accidents are human error, inadequate training of staff and poor maintenance of equipment. The indicated preventive measures are the use of suitable personal protective equipment (P.P.E.) and the adequate training to standard job procedures (e.g. LOTOTO, S.O.P). These measures contributes significantly to prevent accidents. Mitigation of their consequences can be achieved by immediate first aid and isolation of the accident site. | en |
